The Word version supplied by author differed in page layout from the printed version. It is suspected to be a version BEFORE the final version, as there are also some minor differences in the text and bibliography. The layout and obvious changes were edited to conform with the printed version before converting to PDF. The contents page was amended to reflect the page numbering of the printed version. Some footnotes are not synchronised with the text. However, it remains a reasonably faithful reproduction of the printed copy.
