The missing pages 138, 144 and 148 are blank in the original thesis. Thesis contains many images from galleries and collections. Many photos were taken by the author and most of the images are probably OK to include on the web, but pending clarification of this, copies are available only on request. A higher-resolution copy is stored offline.
